On-call premium pay for qualifying shifts.Use your strengths to serve multiple settings – Assisted Living, Independent Living, Home Health, and Outpatient.Spend more time with your residents/clients with reasonable productivity expectations.Paid mileage and drive time if traveling between communities.Access to online continuing education courses & podcasts.Scholarship program – awards up to $5,250 per year!May be eligible for Public Service Loan Forgiveness.Opportunities for growth and internal promotion!ResponsibilitiesAs a Physical Therapist you will be responsible to:Conduct a comprehensive assessment & evaluation using subjective and objective measures.Develop treatment plans and programs in accordance with physician orders.Provide quality treatments, actively progressing goals to achieve highest quality of life.Conduct screens and provide education to residents/clients, families, caregivers, and staff.Coordinate seamless discharges utilizing the full continuum of therapy and fitness services.Provide accurate and timely documentation & billing in electronic medical record.Demonstrate understanding and knowledge of payer sources and regulatory requirements.Complete and document supervisory visits per regulations.Communicate effectively with residents/clients, families, caregivers, physicians, referral sources, and interdisciplinary team.QualificationsGraduate from an accredited Physical Therapy program.Current licensure in the state of practice as required.Competence in Physical therapy evaluation and treatment.Demonstrated compatibility with PHS's mission and operating philosophies.Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.Pay RangeThe estimated starting pay range for this role is $39.67 - $46.67 per hour. This good faith estimate is based upon PHS's typical starting pay range for this role.
