Key Responsibilities

• Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in collaboration with the interdisciplinary team to progress patients toward discharge goals.
• Manage individual, concurrent, and group treatment sessions; coordinate equipment trialing and ordering, patient and family training, home exercise program issuance, and planning for continuing services.
• Complete medical record documentation in a clear, concise, accurate, and timely manner, including daily notes, goal writing, functional quality indicator assessments, letters of medical necessity, discharge documentation, and charge entry in compliance with state, federal, and intermediary guidelines.
• Ensure all care is provided under signed, valid physician orders and within insurance approvals as required for patient care.
• Participate in student clinical education programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations.
• Adhere to discipline-specific state practice acts and all regulatory requirements; maintain professional licensure in good standing and fulfill all state-mandated continuing education requirements.

Required Qualifications

• Active Physical Therapist license in good standing in the state of Arizona
• Completion of all state-mandated continuing education requirements
• Ability to perform comprehensive patient evaluations and develop clinically reasoned treatment plans
• Proficiency in medical record documentation in compliance with state, federal, and payer guidelines
• Demonstrated ability to work collaboratively within an interdisciplinary team environment

Preferred Experience / Skills

• Experience in inpatient rehabilitation or acute care settings
• Prior experience supervising or mentoring Physical Therapy students
• Familiarity with performance improvement methodologies and quality initiatives
• Active involvement in professional organizations or continuing education beyond minimum requirements
