Provides technical nursing and skilled interventions as ordered by the physician. Instructs, motivates, and assists patients / caregivers in learning and improving ability to live independently at home.Job Duties:Ensures physician orders are followed and included in patients plan of careDevelopment and revision of the plan of care based on physician ordersEnsures nursing orders are carried out in a professional manner satisfying professional ethics, patient rights and State and Federal codes and regulationsContinually monitoring patient outcomes and reporting changes to supervisors to prevent hospital readmissionParticipates in effective communication amongst other clinicians providing care to the patientCompletes and submits all required documentation within established agency guidelinesCommunicates the patient's progress to supervisor and make changes as necessary under the physician's directionCovers after hours, weekend and holiday on call to meet the branches needMaintains confidentiality of all company and patient related information.
