POSITION OVERVIEW

The Social Worker provides counseling and clinical services to individuals experiencing substance use disorders and co-occurring behavioral health needs in an integrated outpatient setting. This role maintains an assigned caseload, conducts assessments, develops treatment plans, provides individual and group counseling, and works closely with the interdisciplinary treatment team. As a Clinician I, the Social Worker develops clinical independence, sound judgment, timely documentation, and effective patient care through supervision and professional development.

KEY ESSENTIAL FUNCTIONS

·Maintain an assigned caseload and provide services within the scope of New York State social work licensure, agency policies, and applicable supervision requirements.

·Complete psychosocial and intake assessments, screenings, risk assessments, and other clinical evaluations as assigned.

· Develop, review, and update individualized treatment plans with patients based on their needs, strengths, and treatment goals.

· Provide individual and group counseling using evidence-based approaches, including Motivational Interviewing and Cognitive Behavioral Therapy, as appropriate to training and scope of practice.

·Support patients with substance use treatment, recovery planning, relapse prevention, medication-assisted treatment education, and connections to community resources.

·Monitor patient progress, engagement, attendance, safety concerns, and changes in treatment needs; communicate concerns promptly to the treatment team.

·Coordinate referrals, discharge planning, and access to services that support continuity of care; communicate with outside providers and agencies when authorized.

·Collaborate with clinical leadership, medical and nursing staff, intake staff, support staff, and other clinicians; participate in case conferences, team meetings, and clinical supervision.

· Provide telehealth services when appropriate and consistent with agency procedures.

·Complete accurate, timely assessments, progress notes, treatment plans, and discharge documentation in the electronic medical record, including required OASAS reporting.

·Conduct toxicology testing as assigned and consistent with agency procedures.

·Protect patient confidentiality and comply with HIPAA, 42 CFR Part 2, OASAS requirements, social work ethical standards, and agency policies.

· Complete mandatory training; maintain required licensure and continuing education; meet established productivity, documentation, attendance, and performance expectations.

· Maintain professional boundaries and communication, seek supervision when needed, incorporate feedback, and perform other related duties as assigned.

QUALIFICATIONS

· Master's degree in social work from an accredited program required.

· Current, unrestricted New York State LMSW or LCSW license required

·Knowledge of substance use disorders, co-occurring behavioral health conditions, recovery-oriented care, and evidence-based clinical practices.

·Strong assessment, counseling, documentation, organization, and communication skills; ability to manage a caseload and work with diverse patients and an interdisciplinary team.

·Ability to use electronic medical records and maintain professional boundaries and confidentiality.
