Location: Woodbury, New Jersey Contract Term: March 1, 2026 – February 28, 2027 (1 Year) Work Arrangement: Onsite – Regular Resident Care Sessions & Required Meetings

We're looking for licensed Physical Therapist (PT) to provide comprehensive rehabilitative care to residents. The PT will assess, plan, and deliver therapy services that promote resident mobility, safety, and independence in accordance with federal, state, and facility regulations.

Key Responsibilities: Clinical & Rehabilitative Care

• Conduct comprehensive resident assessments upon admission, readmission, quarterly, and upon significant change in condition.
• Develop and implement individualized physical therapy plans based on functional goals.
• Provide rehabilitation interventions to improve mobility, balance, and independence.
• Attend care planning meetings and weekly fall meetings with the IDCT team; screen residents within 72 hours of any fall.
• Work with the MDS Coordinator to select ARD dates for maximum RUG reimbursement and care optimization.
• Collaborate with nursing staff on Nursing Restorative Programs and participate in monthly review meetings.

Documentation & Compliance

• Complete all documentation using the Sigma Care System for assessments, progress notes, and treatment plans.
• Maintain updated resident medical records and therapy logs.
• Notify the Administrator/Director of Nursing of any therapist changes or service adjustments.
• Attend quarterly rehabilitation team meetings and ensure compliance with all NJ and federal rehab regulations.

Education & Qualifications

• Licensed Physical Therapist in the State of New Jersey (Required)
• Active Malpractice Insurance (Required)
• Experience in long-term care or skilled nursing facilities
• Familiarity with Sigma Care or similar EMR systems
• Strong knowledge of federal and state rehab regulations
