Skills Required:

• Perform comprehensive patient assessments, including medical, psychosocial, and discharge planning needs.

• Develop and coordinate individualized discharge plans.
• Collaborate with physicians and the interdisciplinary care team to facilitate patient progression.
• Coordinate post-acute services including home health, skilled nursing facilities, inpatient rehabilitation, durable medical equipment (DME), and outpatient services.
• Conduct utilization review and ensure compliance with payer guidelines and regulatory requirements.
• Obtain prior authorizations and coordinate insurance approvals.
• Participate in multidisciplinary rounds and patient care conferences.
• Educate patients and families regarding discharge plans and available community resources.
• Maintain accurate documentation within the electronic medical record (Epic preferred).
• Ensure compliance with CMS, Joint Commission, and hospital quality standards.

Requirements

• Current California Registered Nurse (RN) license in good standing.
• Current BLS certification through the American Heart Association.
• Minimum of 2 years of recent Acute Care Hospital Case Management and/or Discharge Planning experience.
• Acute Care Hospital experience required.
