This role focuses on comprehensive medical management, continuity of care, and collaboration with interdisciplinary teams to optimize patient outcomes. Salary Range: $200,000 – $230,000 Key Responsibilities Provide primary medical care to nursing home patients in accordance with standards Assess, diagnose, and manage acute and chronic medical conditions Order and interpret laboratory and imaging studies ; refer to specialists as needed Prescribe medications and treatment plans , including lifestyle and dietary guidance Educate patients on treatment options and medication side effects Maintain accurate, timely, and compliant documentation Collaborate with multidisciplinary teams to ensure optimal patient outcomes Adhere to quality improvement, safety, and infection control standards Requirements MD or DO from an accredited U.S. medical school Board Certified or Board Eligible in Family Medicine, Internal Medicine, Geriatrics, or Palliative Care/Hospice Completion of an accredited U.S. residency (or equivalent pathway) Active New York State medical license (or eligibility during credentialing) DEA registration (or ability to obtain during credentialing) Experience providing primary and diagnostic care in a clinical setting Preferred Skills Experience working with geriatric or long-term care populations Strong collaboration and communication skills within multidisciplinary teams Benefits Benefits Competitive salary: $200,000 – $230,000 Work in a supportive, hospital-affiliated long-term care environment Opportunity to focus on geriatric care and patient continuity Collaborative interdisciplinary team environment ⚡ Professional growth and exposure to long-term care best practicesaa415a4b-8b21-40fc-a65c-70d2b25ca29a
