Position Overview

The following outlines the core responsibilities and essential functions of the role:

Exemplary Practice and Outcomes

• Utilizes the Nursing Process (assessment, diagnosis, outcome identification, planning, implementation, and evaluation) to deliver clinical care grounded in evidence-based practices, employing analytical and critical thinking to ensure personalized care. This involves coordinating care through effective partnerships, recognizing that compassion is key to achieving optimal patient outcomes.
• Delivers patient-centered care that is tailored to specific population characteristics (such as age, gender, and medical conditions), adapting service delivery to reflect an understanding of cultural diversity.
• Engages with patients and their significant others (when appropriate) through suitable methods for communication and collaboration (e.g., bedside shift reports, interdisciplinary rounds, timely patient education), ensuring that patient goals are central to the care plan.
• Adheres to current clinical practice standards.

Teamwork and Collaboration

• Coordinates the provision and documentation of safe, high-quality patient care that supports a professional care delivery model. This includes diverse and inclusive communication methods (such as interdisciplinary rounds and case reviews), timely documentation, and fostering a respectful, inclusive clinical environment.
• Exhibits teamwork and collaboration by practicing in a fiscally responsible manner that encourages the recruitment and retention of all healthcare team members.
• Engages in performance improvement initiatives that include all stakeholders, utilizing research to enhance safe, quality patient care. This may involve initiating or leading activities and actively participating in shared governance at all organizational levels.

Professional Development and Initiative

• Completes all initial and ongoing assessments of professional competency, mandatory education, and population-specific training.
• Acts as a preceptor and/or mentor for fellow nurses and other staff or students across disciplines, ensuring a qualified workforce for the future while modeling professional nursing practices and fostering a healthy work environment.

Evidence-Based Practice and Research

• Advocates for evidence-based practices, nursing research, and performance improvement within nursing. Participates in activities such as value analysis to inform decision-making regarding patient care products.
• Complies with all organizational policies, procedures, and clinical competencies outlined in this job description and by professional associations, including legal, regulatory, and accreditation standards, such as TJC Patient Safety Goals.
• Engages in data collection and formulates relevant clinical questions to advance evidence-based practice, consulting experts and utilizing appropriate resources to address practice inquiries.

Resources and Support

• Proactively plans for patient care across the continuum while providing direct patient services.
• Contributes to the development of protocols and procedures, either upon request or through self-initiation, in collaboration with care managers and other healthcare team members to achieve optimal practice outcomes (e.g., reducing readmission rates, avoidable days, and adverse events).
• Supports the efficient and effective use of both human and material resources.

Education Requirements

• Associate's Degree in Nursing or Diploma in Nursing required; Bachelor's Degree in Nursing preferred.

Licenses and Certifications

• All certifications must be obtained upon hire unless otherwise specified.
• Registered Nurse (Single State) or RN-COMPACT (Multi-state Compact).
• BLS - Basic Life Support or equivalent certification.
• ACLS - Advanced Cardiac Life Support or equivalent certification, with provisional ACLS certification required within 180 days.

Experience Requirements

• A minimum of 2 years of current experience in the specified area of practice is required.
• Experience with computer/data entry is required.
• Familiarity with Epic and 3M systems is preferred.
