RN Case Manager

We seek a highly skilled and experienced Registered Nurse with acute care and case management expertise. The Registered Nurse will provide skilled nursing care, patient assessments, medication administration, treatment planning, and collaborate with the interdisciplinary team. This role requires at least 2 years of relevant experience and the ability to work effectively in a fast-paced environment. The position includes weekend work every other weekend and floating between like units as needed. Responsibilities: Required Skills & Certifications:

• Active Registered Nurse license in the state of practice.
• Minimum of 2 years of acute care nursing experience.
• Strong clinical assessment, medication administration, and treatment planning skills.
• Proficiency with electronic medical records, specifically Epic.
• Excellent communication and interpersonal skills.
• Bachelor's degree in Nursing or related field.
• Driver's license, car insurance, and reliable automobile for field work if applicable.

• Experience in case management and interdisciplinary team collaboration.
• Prior experience in acute care environments such as ICU, ER, or Medical/Surgical units.
• Familiarity with hospice or home health nursing.

• Required to work every other weekend.
• No on-call responsibilities.
• Floating between like units based on staffing needs.
• Facility offers support with housing, including realtor assistance if needed.
• Work environment includes proximity to multiple state and national parks.

• Work schedule includes every other weekend shifts.
• Shift assignments may require floating between units.
• Start date is ASAP, with schedules coordinated to meet facility needs.
