Part Time: 1st, 2nd, and 3rd Shift - 8 or 12hr shifts available.

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Walnut Creek Nursing and Rehab, a 115-bed skilled nursing facility in Erie, Pennsylvania. As a key member of our interdisciplinary healthcare team, you will play a vital role in providing high-quality care to our residents in a comfortable and welcoming environment.
About the Role:
As an LPN at Walnut Creek, you will be responsible for providing direct patient care to our residents, including administering medications, monitoring vital signs, and performing routine nursing tasks. You will work closely with our registered nurses, therapists, and other healthcare professionals to develop and implement individualized care plans that meet the unique needs of each resident. Your excellent communication and interpersonal skills will enable you to effectively collaborate with residents, families, and other healthcare team members to ensure seamless care delivery. Available for a limited time only is a $5,000 Sign-On Bonus!
Responsibilities:
• Provide direct patient care to residents in accordance with established nursing standards and protocols
• Administer medications, monitor vital signs, and perform routine nursing tasks as needed
• Develop and implement individualized care plans in collaboration with other healthcare team members
• Communicate effectively with residents, families, and other healthcare team members to ensure seamless care delivery
• Participate in interdisciplinary care planning and coordination
• Maintain accurate and up-to-date records of patient care

Requirements:
• Current LPN licensure in the state of Pennsylvania
• Experience in a long-term care setting preferred
• Ability to work in a fast-paced environment with a focus on quality patient care
• Excellent communication and interpersonal skills
• Ability to maintain confidentiality and adhere to HIPAA regulations
