Habitat Health is seeking an Intake Coordinator to facilitate the intake process for prospective participants entering our PACE program. You will coordinate home visits, center tours, assessments, and enrollment steps, partnering with caregivers and cross-functional teams to ensure a smooth, participant-centered experience. You'll maintain participant information in CRM/EMR, support documentation, and coordinate with Outreach, Enrollment, and clinical pre-enrollment teams to enable timely #J-18808-Ljbffr habitathealth.com
