Organization VCH Step Down Job Summary Responsible for planning, organizing, and administering patient care within the scope of his/her training and expertise, collaborating with other professional disciplines to ensure effective and efficient patient care delivery and achievement of desired patient outcomes. This is done in conjunction with the objectives and policies of the hospital and the Nurse Practice Act of the State of Tennessee. Hours of Position Full-time (36 hours/week), 6:45p-6:45a Stepdown is a 21 bed hardwired telemetry unit that focuses on acutely ill but stable patients requiring closer monitoring. We care for patients with a variety of cardiac, vascular and medical diagnoses with specialized needs such as cardiac arrhythmias, bedside cardioversions, titrated medications, oxygenation management, withdrawal, sepsis, stroke, and post-operative care. Our staff is dedicated to providing excellent care to our patients. The environment is challenging, fast-paced, team oriented, and patient and family centered. Staff are engaged in problem‑solving and identifying ways to improve patient care. They have strong communication skills and collaborate with the team to provide excellent care. Daily interdisciplinary rounds include the provider, registered nurse, case manager, and a nurse leader. Each shift begins with a safety huddle to review safety and quality concerns for the next 24 hours. The patient‑to‑nurse ratio is 4:1. Certifications required: BLS, ACLS, NIH. Orientation: 8-12 week orientation based on nursing experience. Professionalism: critical thinking, collaboration, multidisciplinary rounding, bedside shift report, shared governance, certifications. Medical equipment used: ECG hardwired, alaris, PCA, epidural, EtCO2, bladder scanner, BiPAP, high‑flow oxygen therapy. Drip medications include cardiac, IV fluids, blood products, sedation, TPN. Key Responsibilities Collaborate in establishing the plan of care, coordinate and implement care delivery while anticipating patient care needs across the continuum. Analyze comprehensive information pertinent to the patient’s care and management to determine nursing diagnosis. Continually utilize the nursing process to evaluate the patient’s progress toward goals and apply appropriate interventions to ensure effective outcomes. Employ strategies to promote health and a safe, healthy environment. Additional duties may be assigned as needed. Technical Capabilities Patient assessment & evaluation in home care (novice): demonstrates ability to assess and evaluate patients in moderate difficulty home settings, conduct primary care interviews, assess home safety, and perform comprehensive physical assessments. Patient education (novice): provides patient education in moderate difficulty applications. Care planning (novice): anticipates resource needs, identifies tasks, sets priorities, schedules activities, meets deadlines, and organizes work within direct responsibility. Prioritizes tasks and resolves issues with minimal disruption. Evidence‑based nutrition practice (novice): searches for nutrition evidence to answer clinical questions, works under direction while accomplishing assignments.
