Registered Nurse Care Manager

SpringVale Health Centers, Inc. is seeking a Registered Nurse for a Care Manager position to provide a variety of nursing duties in a Medical office setting. This is a full-time position based in our Dover medical office.

Key Responsibilities:

• Conduct initial and periodic holistic assessments of patients, including physical and psychological evaluations.
• Maintain ongoing telehealth relationships with patients/members in a Care Management relationship.
• Develop and implement individualized care plans based on patient assessments.
• Collaborate with physicians, nurses, and other healthcare professionals to ensure seamless care delivery.
• Facilitate patient access to community resources and support services to enhance care and promote independence.
• Participate in quality improvement initiatives to enhance care delivery and patient outcomes.
• Maintain contracted payer relationships.
• Assist Nursing Coordinator with coverage of duties requiring an RN in their absence.

Minimum Requirements:

Bachelor's degree in nursing and current license to practice in Ohio as a Registered Nurse. Minimum of three (3) years of professional nursing experience, preferably in care management, case management, or a related field. FQHC experience and knowledge of HRSA, CARF, CPC, and VBC are highly preferred.
