Job Requirements We are seeking a dedicated Palliative Care Nurse Practitioner to join our collaborative team and provide high‑quality, patient‑centered care focused on improving quality of life for individuals facing serious illness. As a member of our Palliative Care team, you will: Provide expert palliative care consultations for patients with serious and complex illnesses Partner with patients, families, and interdisciplinary teams to align care plans with patient goals and values Support symptom management, advance care planning, and shared decision‑making Collaborate with physicians, nurses, social workers, chaplains, and other healthcare professionals Help expand access to compassionate, high‑quality palliative care services What Makes This Opportunity Unique?
