Role Description This full-time (or could consider part-time), on-site (maybe some virtual) Licensed Practical Nurse / Licensed Vocational Nurse (LPN / LVN) role in Fort Worth, TX. The LPN will provide direct patient care under the supervision of physicians and APPs, including call triage, administration of value based programs, medicare chronic care management programs, patient interactions and other nursing tasks. Qualifications Active, unencumbered Licensed Practical Nurse / Licensed Vocational Nurse (LPN/LVN) license in the state of Texas.
