I Major Responsibilities: Supportdailydischargeplanningbycoordinatingandcommunicatingwithcareteams,identifyingpostacuteoptions,and updating Estimated Discharge Date (EDD) and placement details.

1. Initiate,coordinate,andtrackreferralsandtransfersasassignedwhilemaintainingaccuratedocumentationinrequiredsystems.
2. Completeessentialdischargeworkflowsandregulatorysteps,includingMedicarenoticesandcompliancetasksasassigned.

1. Coordinatepostacuteservicessuchasrehab,SkilledNursingFacilities(SNF)placement,VistingNurseAssociation(VNA), Durable Medical Equipment (DME), and MassHealth forms.
2. Identifyandescalatedischargebarriers,includinginsurance,transportation,orplacementdelays.
3. Maintaineffectiverelationshipswithpostacuteprovidersandensureuptodateresourcelistsforcareteams.
4. Facilitateaccesstobehavioralhealth,substanceusetreatment,housingsupports,andcommunityresources.
5. Assistwithinsurancecoordination,prescriptionsupport,formpreparation,collateralgathering,andnotaryneeds as required.

All responsibilities are essential job functions. II Position Qualifications: License/Certification/Education Required:

1. Bachelor'sdegree
2. Onetothreeyears' experienceinahealthcaresettingassistingwithdischargeplanning,carecoordination support, or a related health science role
3. BasicLifeSupport(BLS)certificationactiveorwithin30daysofhire
