Physical Therapist Assistant (PTA) - Home Health ????✨
Full-Time | Day Shift
???? Location: New Stanton, PA
???? Schedule: Monday-Friday | 8:00 AM - 4:00 PM
Community Care, Inc. is looking for a compassionate and dedicated PTA to provide home health care to our clients in the Westmoreland and Fayette County area. This is a great opportunity to build meaningful connections, deliver personalized care, and see the impact of your work every single day-all in a supportive home setting.
???? What You'll Do:- Provide physical therapy treatment plans as established by the supervising Physical Therapist
- Provide patient education and instruction in home exercise programs
- Monitor, document, and report patient progress and any changes in condition
- Support patient in achieving functional goals that enhance safety and independence
- Collaborate with a multidisciplinary team to ensure continuity of care
- Document visits and patient progress accurately and timely
- Current Physical Therapist Assistant (PTA) license in the state of Pennsylvania.
- Graduate of an accredited Physical Therapist Assistant program
- Minimum 1 year of clinical experience (Home Health experience preferred)
- CPR certification (required)
- Reliable transportation and a valid driver's license
