(APP) Nurse Practitioner/Physician Assistant - Onion Creek - xpress Care

Position Overview Harbor Health is seeking a dedicated Nurse Practitioner or Physician Assistant to join our multi-specialty clinic group at our Onion Creek location in Austin, TX. As a clinical leader within our primary care setting, you will provide effective, efficient, and appropriate services to our members while demonstrating safe clinical decision-making and expert care. You will work collaboratively with our clinical practice team to deliver exceptional care that aligns with our mission of transforming healthcare through our payvider model. The APP will provide acute and urgent care services to members across a broad range of presentations, ensuring timely, compassionate, and evidence-based treatment in an Express Care setting with the potential to cover one Saturday/month. Position Duties & Responsibilities Evaluate, diagnose, and treat members presenting with acute, urgent, and episodic medical conditions in an Express Care setting Obtain comprehensive medical histories and perform thorough physical examinations Order and interpret diagnostic tests, laboratory results, and imaging as clinically appropriate Develop and implement individualized treatment plans in alignment with evidence-based clinical guidelines Prescribe medications and therapeutic interventions within scope of practice Educate members and their families on diagnoses, treatment plans, medications, and follow-up care Collaborate with the broader Harbor Health care team to ensure appropriate referrals, care coordination, and continuity of care Accurately and efficiently document all clinical encounters in the electronic health record (EHR) system Adhere to all regulatory, compliance, and credentialing requirements Contribute to a culture of continuous improvement, member safety, and clinical quality Desired Professional Skills & Experience Current licensure as a Nurse Practitioner (NP) or Physician Assistant (PA) in the state of Texas Board certification required (AANP, ANCC, NCCPA, or equivalent) DEA license and prescriptive authority in Texas Minimum of 1–2 years of clinical experience in urgent care, emergency medicine, family medicine, or a related ambulatory care setting preferred Strong clinical assessment and diagnostic reasoning skills Experience working with diverse member populations across age groups Proficiency with electronic health record (EHR) systems Excellent interpersonal and communication skills with an ability to build trust with members quickly Ability to work independently and collaboratively within a multidisciplinary team Commitment to Harbor Health's mission of transforming healthcare in Texas What We Offer Opportunity to shape the day-to-day member experience and operational success of a Harbor Health clinic Collaborative and dynamic work environment An organization made of people who are passionate about changing the healthcare landscape Competitive salary and benefits package Professional development and growth opportunities A transparent and unique culture Company Overview At Harbor Health, we're transforming healthcare in Texas through collaboration and innovation.
