VibrantCare Rehab in Citrus Heights, CA is seeking a dedicated Physical Therapist (PT) to deliver exceptional, patient-centered rehabilitation services. The ideal candidate will evaluate and treat patients, develop personalized care plans, and collaborate closely with patients, families, and healthcare teams to promote optimal recovery and quality of life. Responsibilities

• Evaluate and treat patients requiring skilled physical therapy services.
• Develop individualized treatment plans and monitor patient progress.
• Document evaluations, treatments, goals, progress, and medical necessity accurately and promptly.
• Supervise and direct Physical Therapist Assistants and aides as appropriate.
• Communicate effectively with patients, families, physicians, coworkers, and facility staff.
• Participate in facility meetings, case management, and clinical in-services.
• Maintain compliance with state regulations, Medicare requirements, OSHA standards, and company policies.
• Maintain required continuing education and current CPR certification.
• Maintain positive relationships with facilities, clients, and healthcare partners.
• Meet productivity and quality-of-care expectations.

Required Qualifications

• Graduate of a CAPTE-accredited Physical Therapy program or substantially equivalent international education.
• Current California Physical Therapist license/registration as required.
• Current CPR certification.
• Current TB test, maintained according to state guidelines.
• Strong clinical problem-solving and communication skills.
• Excellent organizational, time-management, and planning abilities.
• Ability to work effectively as part of a team.
• Leadership skills to supervise and support PT assistants and aides.
• Strong computer and clinical documentation skills.

Physical Requirements

• Ability to frequently lift and assist patients using appropriate biomechanical techniques.
• Ability to lift up to 50 lbs. independently.
• Ability to walk, bend, stand, and reach throughout the workday.
• Ability to push wheeled equipment.
• Sufficient visual, hearing, and fine motor abilities to perform clinical and documentation duties.

Compensation & Benefits

• Base Salary: $80,000–$90,000 per year
