Physical Therapist Assistant (PTA) - Assisted Living Facility | Port Jervis, NY | Full-Time or Part-Time

Stern at Home Therapy is hiring a Full-Time or Part-Time Physical Therapist Assistant (PTA) to provide one-on-one physical therapy treatments at an established Assisted Living Facility (ALF) in Port Jervis, NY .

This is an excellent opportunity for a PTA looking for the consistency of a facility-based caseload without the travel requirements of traditional home health . Patients are treated at one established facility, allowing you to spend your time providing care rather than driving between appointments.

You'll work under the supervision of licensed Physical Therapists who establish each patient's plan of care and provide ongoing clinical oversight.

What You'll Do:

As a Physical Therapist Assistant with Stern at Home Therapy, you'll provide individualized physical therapy treatments to a primarily geriatric patient population within the assisted living facility.

Responsibilities include:

• Provide one-on-one physical therapy treatments at a single assisted living facility.
• Carry out individualized plans of care established by the supervising Physical Therapist.
• Help patients improve strength, balance, mobility, endurance, safety, and overall functional independence.
• Provide therapeutic exercise, gait training, transfer training, balance activities, and other interventions outlined in the plan of care.
• Monitor patient progress and communicate changes in condition to the supervising Physical Therapist.
• Educate patients and caregivers on exercises, mobility strategies, and safety.
• Complete accurate and timely clinical documentation.
• Communicate with patients, families, facility staff, supervising PTs, and interdisciplinary care team members.
• Coordinate with Stern's administrative team regarding scheduling and patient care.

PTAs do not perform evaluations.

Compensation

Full-Time - Minimum 16 Patients:

Full-Time W-2:

• $35-$50 per visit.
• Tiered compensation structure with the opportunity to increase your per-visit rate as patient volume grows.

Full-Time 1099:

• $50-$60 per visit.
• Tiered compensation structure with annual earning potential up to $105,300 .

Part-Time - Minimum 10 Patients:

Part-Time W-2:

• $35-$50 per visit.
• Fixed per-visit compensation with no tiered volume model.

Part-Time 1099:

• $50-$60 per visit.
• Fixed per-visit compensation with no tiered volume model.

Qualifications:

• Active Physical Therapist Assistant license in New York required.
• Graduate of an accredited Physical Therapist Assistant program.
• Strong clinical, communication, documentation, and time-management skills.
• Ability to work independently while collaborating with supervising Physical Therapists and facility staff.
• Geriatric, assisted living, skilled nursing, outpatient, or home health experience preferred but not required.
