Location: San Diego and all surrounding areas

Schedule: Mon- Friday with Weekends Rotation

As a Home Health MSW, you will:

• Assess patients’ social, emotional, and environmental needs and identify appropriate community resources, in-home support services, and other available assistance.
• Collaborate with physicians, case managers, and interdisciplinary team members to incorporate social and emotional needs into the patient’s Plan of Care (POC).
• Provide counseling, education, and support to patients and families/caregivers regarding social needs, community resources, advance directives, and available services.
• Connect patients and families with appropriate community resources and coordinate services to support patient well-being and independence.
• Monitor and communicate changes in patients’ social and support needs to the care team and document relevant findings in the patient health record.
• Participate in discharge planning and care coordination to support a safe and effective transition from home health services.
• Maintain accurate, timely, and confidential electronic clinical documentation in accordance with Lorian policies, HIPAA, and applicable regulations, while participating in continuing education and in-service programs.

Required Qualifications

• Graduate of an accredited school approved by the Council on Social Work Education.
• Master's degree from a school accredited or approved by the council on Social Work Education.
• Valid driver's license and insured personal vehicle (per Lorian requirements)
• Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.

This position is based on a pay per visit compensation model. Pay per visit ranges from $40 to $100 visit commensurate with experience and depending on type of visit.
