Care Coordinator - Behavioral Health Clinic (Full Time - Tyler) Are you passionate about helping others connect to the care and resources they need? You'll help clients navigate referrals, coordinate aftercare with hospitals, and ensure smooth transitions so individuals can focus on what matters most: their wellness and recovery.
