What You’ll Do As a Home Health Physical Therapist, you will:
Perform comprehensive patient evaluations and develop individualized treatment plans
Deliver skilled physical therapy services to improve mobility, strength, balance, and overall function
Help patients manage pain and prevent further physical limitations
Monitor patient progress and adjust treatment plans to maximize outcomes
Educate patients and families on exercises, safety, and disease management
Collaborate closely with physicians, nurses, therapists, and interdisciplinary care teams
Maintain accurate and timely documentation in accordance with company guidelines
Uphold infection control and patient safety standards during every visit
What We’re Looking For Graduate of an accredited Physical Therapy program
Current Physical Therapy license
CPR Certification
Valid driver’s license and auto insurance
Home health experience preferred
HomeCare HomeBase experience preferred
Strong communication, clinical, and time management skills
Passion for delivering patient-centered care in the home setting
