Social Services Designee – Skilled Nursing Facility Harmony Chicago, a Gold Seal of Approval® certified post‑acute care facility located in Albany Park, Chicago, seeks a Social Services Designee to support residents’ social and emotional needs. Key Responsibilities Prepare residents for transitions and discharges. Coordinate care conferences and discharge planning. Provide ongoing social and emotional support to residents. Work collaboratively with interdisciplinary team. Qualifications High school diploma or equivalent. Minimum 2 years experience in a Social Worker role, preferably in a healthcare setting. Prior experience as a Social Services Designee in a skilled nursing facility preferred. Bachelor’s degree in Social Work or related field and 5 years related experience preferred. Benefits Daily pay.
