Your next nursing opportunity starts here. We are seeking a compassionate and skilled Registered Nurse with expertise in home health and OASIS documentation to fill a 13-week full-time contract. This role allows you to deliver personalized care in patients homes and collaborate with a dynamic, professional care team.

Schedule:

• 13-week contract, with the possibility of extension
• Full-time hours, Monday through Friday
• On-call responsibilities are probable

Key Responsibilities:

• Deliver high-quality in-home nursing care and skilled clinical interventions
• Conduct comprehensive patient assessments and complete accurate, timely OASIS documentation
• Implement physician-ordered care plans through effective collaboration with physicians and caregivers
• Perform wound care, medication administration, patient education, and chronic disease monitoring
• Clearly communicate clinical updates and changes in patient status to the interdisciplinary team

What We Are Looking For:

• Current RN license in good standing
• At least one year of recent experience as a home health RN
• Demonstrated OASIS proficiency
• Strong knowledge of HomeCare HomeBase (preferred)
• Comfortable managing a full-time, structured daily visit schedule
• Reliable transportation and a valid drivers license
