Registered Nurse - PRN

Role Overview As an Extended Care Team (ECT) Nurse, you will be a vital support member of the interdisciplinary care team, providing flexible, high-quality nursing care in hospice settings. Your primary responsibility is to assess, treat, and communicate patient needs promptly and accurately, ensuring no need goes unmet. Key Responsibilities Deliver comprehensive nursing care within your scope of practice Conduct patient and family assessments, contributing to personalized care plans Perform various visit types to meet patient and family needs Administer medications and treatments as ordered Identify nursing diagnoses and respond appropriately Document clinical notes accurately, including pain and symptom management Communicate patient condition changes to physicians and obtain necessary orders Coordinate with community health partners for seamless care transitions Ensure equipment and services are available and arranged Maintain and develop clinical skills through ongoing education Qualifications & Skills Active RN license in the relevant state Graduation from an accredited nursing program Experience in hospice, home health, or related fields preferred Flexibility to work across varied patient assignments Strong assessment skills and sound clinical judgment Excellent documentation and communication abilities Valid driver’s license and willingness to travel locally #J-18808-Ljbffr
