The LPN will provide care in a long‑term care setting under the supervision of a registered nurse and will rotate through day/evening/night schedules as needed. Collect data on residents’ physical and psychosocial status and communicate findings to the RN and interdisciplinary team. Contribute to the development and revision of an age‑appropriate, culturally relevant plan of care; involve the resident/family and healthcare team. Collaborate with physicians, staff, and ancillary teams to ensure comprehensive care and timely referrals as needed. Maintain current knowledge and competency in nursing practice; participate in orientation and staff mentoring as directed. Demonstrate knowledge of growth and development principles for adult and geriatric patients; adhere to safety, infection control, and documentation standards.
