Schedule - Shift - Hours PRN - Days - 8am-5pm Citizens Memorial Hospital (CMH) is hiring alicensed Physical Therapist for our Home Health Department. CMH is made up of 34 primary and specialty clinics, 6 Long-Term Care facilities and 1 residential care facility. Offering acute care, emergency services, ambulatory care, pharmacies, walk-in clinics, surgeries, addiction recovery, therapies and more, CMH has its community covered! About the area: Located in the heart of the Ozarks, our main campus in Bolivar has a hometown feel and affordable housing options of rentals or apartments. About the department: The goal of the Home Health Department is to decrease the number of hospital days and hospital visits per patient by providing skilled nursing and other Home Health services in the patient’s home. Also, to instruct and assist patients and their families regarding their diagnosis, new procedures, medications, prevention of complications, control of pain, and general home care. Individualized plans of care are developed and used to assess, intervene, and treat home-bound patients who may not otherwise receive the healthcare they need due to physical, mental, or socio-economic circumstances. A typical day includes: Traveling to the homes of patients to assess conditions and treatment plan progress, and providingtherapies aimed at improving mobility andrecovery.You will manage your own daily schedule of appointments, creating autonomy and flexibility for you in this role. Establishan effective plan of care using appropriate skills for evaluation and screening Developand implementtreatment plans that lead to optimal patient outcomes Communicateeffectively with patients, care team members, physicians, and others Writeclear, concise, legible documentation that includes appropriate information for care planning Thisposition is: PRN days, 8am-5pm Job Requirements: Education:Graduation from an accredited college with a Doctorate in Physical Therapy Experience:New graduates welcome; 1 year of experience preferred Skills: Patient care Licensure/Certification:Licensed by the Missouri Board of Registration for the Healing Arts Physical:Prolonged periods of standing and walking, ability to lift, push, and pull up to 50 lbs. with patient and/or equipment assistance, ability to carry up to 35 lbs.
