Overview Under the direction of the Assistant Director of Pharmacy Clinical Services, Assistant Director of Pharmacy Operational Services, and Director of Pharmacy Services, the Medication Safety Pharmacist serves as the Medication Safety Leader at Antelope Valley Medical Center (AVMC). This role also represents the pharmacy in opioid stewardship and substance use disorder services. Responsibilities include administrative, regulatory reporting, education, and teaching duties; overseeing the medication safety plan; developing and implementing error-prevention strategies; participating in organization-wide medication use technology design and implementation; overseeing processes regarding medication errors and system failures; and ensuring compliance with state and national standards. Clinical functions include patient care related to drug therapy, reviewing and developing drug protocols for acute on chronic pain or substance use disorder in the inpatient setting, and providing additional clinical services to the healthcare team and the public. The specialist conducts literature-informed evaluation of medical information and supports drug therapy decisions, providing consultative clinical pharmacy services to patients and health care providers and ensuring optimal pharmaceutical care. Duties and Responsibilities Perform duties and functions of a clinical pharmacist specialist in compliance with California and Federal pharmacy laws and regulations. Oversee the activities of pharmacy technicians and interns; assist in orientation and training of department personnel. Oversee medication safety processes, including: Serve as chair for the Medication Error Reduction Plan (MERP) committee. Act as liaison between the quality team and pharmacy services. Act as the subject matter expert in medication safety throughout the organization. Perform medication safety rounds. Assess adverse drug events and medication errors and develop action plans, including reporting to regulatory bodies as necessary. Precept pharmacy residents, graduates, and students in clinical clerkships. Maintain professional work habits and organize workload; prioritize tasks; use resources effectively; comply with program standards; adapt to change. Obtain medication histories, maintain and monitor medication profiles; assist with formulary management and dissemination of drug information. Provide primary care and consultative services to patients and providers to ensure optimal drug therapy. Review medication orders for accuracy, rationality, and cost effectiveness (drug choice, indication, route, and rate). Assess patient drug therapy; perform pharmacokinetic analyses and adjust dosing as needed, in consultation with patients and providers. Develop programs to increase pharmacist involvement in rational drug therapy, including monitoring protocols and in-services on medication safety, opioid stewardship, and substance use disorder therapy. Participate in protocol/guideline development, policies and procedures, and staff development programs for pharmacy and other providers. Engage in medication use evaluation (MUE), quality-focused reviews, ADR activities, and Pharmacy and Therapeutics Committee-directed initiatives. Participate in appropriate committees and maintain clinical expertise through literature reviews and professional involvement. Non-Essential Duties Other duties as assigned. Knowledge, Skills and Abilities Knowledge of pharmacy systems; full understanding of state and federal laws related to pharmacy practice; up-to-date knowledge of drug therapies, interactions, pharmacokinetics, and population-specific needs. Experience with performance improvement methodologies and ability to monitor projects and measure progress. Excellent written and verbal communication; ability to collaborate with health care professionals; strong analytical skills for problem solving and data interpretation. Ability to develop and implement medication safety programs, policies, and protocols; create innovative solutions; training capability for staff across departments. Education and Experience Doctor of Pharmacy degree from an accredited School of Pharmacy. Prefer 1 year of residency (PGY1) or equivalent acute care clinical pharmacy experience; prefer PGY2 residency or fellowship in medication safety or equivalent inpatient medication safety experience. Required Licensure and Certifications Current State of California Pharmacist License or eligibility to be licensed in California within 90 days of hire. Current BLS certification (Red Cross requirements) or obtain within 30 days of hire. Current ACLS certification or obtain within 30 days of hire. Physical Requirements and Working Conditions Office environment; ability to stand, sit, or walk for extended periods; light lifting; typing and use of automated equipment.
