Key Responsibilities

• Conduct thorough assessments of patients' physical conditions, utilizing your clinical expertise to create individualized treatment plans that outline clear and achievable therapeutic goals.
• Implement a variety of evidence-based therapeutic modalities, including but not limited to manual therapy techniques, therapeutic exercises, and modalities such as ultrasound and electrical stimulation, tailored to the unique needs of each patient.
• Actively monitor and reassess patient progress throughout their rehabilitation journey, adjusting treatment plans based on their evolving needs and responses to therapy.
• Educate patients and their families about their conditions, effective treatment strategies, and self-care techniques, empowering them to take an active role in their recovery through home exercise programs.
• Maintain comprehensive and accurate documentation of treatment sessions, patient progress, and outcomes, ensuring compliance with organizational standards and regulatory requirements.
• Collaborate effectively with interdisciplinary healthcare teams, contributing to holistic patient care through regular communication and joint planning.
• Build and nurture trust-based relationships with patients, providing motivation and encouragement to inspire confidence as they work towards their rehabilitation goals.

Patient Population / Client Focus

Your practice will primarily focus on patients in home care environments, ranging from individuals recovering from surgery to those managing chronic conditions. You will support a diverse patient demographic, providing care that addresses various physical challenges that may hinder their mobility and functionality in daily life.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• A valid state licensure or eligibility for licensure as a Physical Therapist in California.
• In-depth knowledge of human anatomy, physiology, and movement sciences as they relate to rehabilitation practices.
• Excellent communication skills that enable effective interaction with patients, families, and members of the healthcare team.
• Proven ability to work autonomously while also contributing to a collaborative team dynamic.

Preferred Experience

• At least 2 years of experience as a physical therapist, preferably in a home care or outpatient setting.
• Experience working with specific populations, such as pediatrics or post-operative patients, is highly valued.

Compensation and Benefits
