TTF is seeking a Behavioral Health Case Manager to coordinate care and support adults working toward recovery from mental health and/or substance use challenges. This hybrid position combines office-based, community/home-based, and telecommute work.
The Case Manager will develop individualized service plans, connect members with behavioral health, medical, and community resources, coordinate with treatment teams and outside providers, provide advocacy and crisis follow-up, and maintain timely documentation.
Candidates must meet Behavioral Health Technician (BHT)qualifications, which may be met through a bachelor’s or master’s degree or qualifying combinations of education and behavioral health experience. A valid driver’s license, auto insurance, and a three-year driving record meeting agency requirements are required. Candidates must also possess or be able to obtain an Arizona Fingerprint Clearance Card and CPR/First Aid certification within 90 days of hire.
