- 8hrs shift, 12:00 AM to 08:30 AM
- 40 hours work week
- Will be needed to work on weekends Sun-Sat w/holidays
Duties & Responsibilities
- Coordinate and manage patient care plans to support quality outcomes and continuity of care.
- Assess patients' needs and develop individualized care plans based on clinical conditions and available resources.
- Monitor patient progress and adjust care plans as needed.
- Collaborate with physicians, nurses, healthcare providers, patients, and families to coordinate appropriate services.
- Monitor utilization of healthcare services and help ensure appropriate and cost-effective care.
- Maintain accurate and timely documentation of assessments, care plans, interventions, and patient outcomes.
- Identify potential barriers to care and assist patients in accessing appropriate resources and services.
- Communicate patient status and care needs effectively with the healthcare team.
- Ensure patient care activities are performed in accordance with applicable policies, procedures, and professional standards.
- Perform other related duties as assigned. .
- Bachelors Degree in Nursing or Associate of Science in Nursing
- Oregon & WA RN or compact license active and on file
- Must have 1 year of case manager or PCC experience
- Previous KP experience preferred
- EPIC charting and experience with Medicaid/Medicare.
- Additional certifications may be required
- Copy of current, completed Primary Source Verification (PSV) from the appropriate licensing board or verification agency, if required.
- Copy of all current required certifications and compliance documents (e.g., . BLS, N95 fit test, immunization records, TB test, or other client-specific requirements ) .
