Behavioral Health Care Manager

Providing telephonic coaching and educational resources to individuals with chronic health conditions, the full-time Compact RN Behavioral Health Coach will assess participant needs, coordinate care, and promote wellness in a 100% remote environment. Key responsibilities Deliver telephonic coaching and referral services to program participants managing chronic health conditions Assess participant needs and communicate with healthcare professionals for effective care coordination Document participant activities and coaching sessions in the case tracking software Required qualifications Nursing Degree from an accredited college or university Active and unrestricted Compact Registered Nurse License 2+ years of experience in direct participant coaching Intermediate knowledge of Microsoft Office and database software
