- Coordinate and manage collaborative care services for residents.
- Work closely with psychiatric providers and licensed social workers to ensure seamless care delivery.
- Monitor and support residents' progress in achieving mental health goals.
- Maintain accurate records using electronic medical records (EMR) systems and provide feedback to the care team.
- Bachelor's degree in psychology, nursing, social work, or a related field.
- Strong organizational and communication skills.
- Ability to work effectively within a multidisciplinary team.
- Proficiency in using electronic medical records (EMR) systems.
- Passion for improving behavioral health outcomes.
- Supervision and guidance provided by licensed social workers, PMHNP, and Psychiatrist
