Homeless Services Case Manager

A community health organization in Arizona seeks an experienced SSVF Case Manager to assist clients in identifying needs and coordinating services. Candidates should have at least a Bachelor's or an Associate's degree in Human Services along with experience working with the homeless population. They will be responsible for developing case management plans, liaising with community resources, and conducting visits to Veterans.
