• Job Setting: Emergency Department - Level III Trauma Center;
• Credentialing: 90 days

• Board Certified/Board Eligible: Board Certification required and must be in hand at time of presentation
• Licensure: Active Indiana state license required and must be in hand; active DEA for the state of Indiana required and must be in hand; CSR if required by state
