Role Overview We are seeking a compassionate, detail-oriented Field Case Manager to support patients and collaborate with a multidisciplinary clinical team. This role involves direct patient interaction, care coordination, advocacy, and field-based travel. Key Responsibilities Coordinate and manage patient care plans in collaboration with the clinical team Advocate for patients and monitor health status, reporting new or changing conditions Maintain accurate, timely documentation and case notes Communicate effectively with patients, families, providers, and internal stakeholders Manage multiple cases while maintaining organization and attention to detail Travel to client locations (up to 80 miles as needed) Work effectively under pressure in dynamic, real-world environments Minimum Qualifications (Must-Haves) Bachelor’s degree (any field) OR: Licensed Practical Nurse (LPN) or Registered Nurse (RN) accepted in lieu of degree Strong communication, organization, and teamwork skills Demonstrated integrity, accountability, and reliability Compassionate and patient-focused mindset with strong advocacy skills Ability to manage responsibilities in a fast-paced, high-pressure environment Valid driver’s license, reliable vehicle, and active auto insurance Willingness and ability to travel regularly for patient visits (up to ~80 miles) Authorized to work in the U.S. Preferred Qualifications (Nice-to-Haves) Degree in healthcare, social services, or related field 1–2+ years of experience in healthcare or case management Experience working with: Traumatic Brain Injury (TBI) Spinal cord injury patients Case management or patient-facing experience (hospital, clinic, home care, or similar) Bilingual or multilingual abilities Our Core Values Honor – Uphold ethical standards and do what is right for patients and the team Openness – Communicate transparently and collaborate effectively Respect – Treat patients, families, and team members with dignity and empathy Innovation – Think critically and adapt to new challenges in patient care Zeal – Bring energy, passion, and commitment to making a difference Ownership – Take accountability for outcomes, decisions, and responsibilities Nurturing – Support and advocate for patients while fostering team growth Growth & Certification Expectations Employees are expected to pursue professional certifications such as: Certified Brain Injury Specialist (CBIS) Eligibility and preparation may begin after the first year of employment Company may cover certification costs once (conditions apply) What We’re Looking For We value individuals who: Demonstrate our Core Values in action Show strong judgment through real-life or situational experiences Exhibit consistency, commitment, and professionalism in their work history Are eager to learn, grow, and contribute long-term Additional Notes Candidates with healthcare experience can be considered even if their degree is in another field Recent graduates with relevant degrees are encouraged to apply (training provided) #J-18808-Ljbffr Horizonccm
