MDS Coordinator

THRYVE Healthcare, a leading healthcare organization, is seeking a highly skilled and dedicated MDS Coordinator to join our team. As an integral member of our clinical staff, the MDS Coordinator will play a critical role in ensuring the highest quality care for our residents while maintaining compliance with industry regulations and standards.

Responsibilities:

• Coordinate and conduct resident assessments, including the Minimum Data Set (MDS) 3.0 assessment, to ensure accurate and timely completion of assessments

• Develop, implement, and maintain resident care plans in collaboration with the interdisciplinary team

• Utilize electronic medical records (EMRs) to document and track resident information and assessments

• Participate in Care Plan Meetings and other interdisciplinary team meetings to ensure resident-centered care

• Collaborate with other departments, such as Nursing, Therapy, and Social Services, to develop and implement individualized care plans

• Identify and report potential quality of care issues and opportunities for improvement to the Director of Nursing and other stakeholders

Requirements:

• 2 years of experience as an MDS Coordinator (or equivalent experience in long-term care or acute care setting)

• Strong knowledge of MDS 3.0 assessment and care planning processes

• Excellent communication and interpersonal skills, with the ability to work effectively with interdisciplinary team members

• Strong organizational and time management skills, with the ability to prioritize tasks and meet deadlines

• Ability to maintain confidentiality and handle sensitive information with discretion
