Key Responsibilities\n

• Conduct comprehensive assessments of patients' needs, strengths, and challenges, enabling the development of personalized treatment plans aimed at promoting patient wellbeing and symptom relief.

• Facilitate psychotherapy sessions for individuals, families, couples, and groups based on evidence-based practices and clinical appropriateness.

• Collaborate closely with the psychiatry team and other healthcare professionals to plan effective care programs that address the multifaceted needs of patients.

• Regularly evaluate and adjust treatment plans by assessing the ongoing biopsychosocial needs of patients, ensuring alignment with their evolving requirements.

Patient Population / Client Focus\n

Your primary focus will be on hospice patients, most of whom are elderly individuals facing serious and terminal conditions. Qualifications\n

• A valid and active Licensed Clinical Social Worker (LCSW) license in Pennsylvania.

Compensation and Benefits\n

This role offers a competitive annual salary ranging from USD 90,000 to 100,000, commensurate with experience.
