Requirements:

• Active Pennsylvania state licensure required (90-day licensure window applies)
• PA DEA registration with Pennsylvania address (excludes CRNAs)
• National Practitioner Data Bank (NPDB) self-query within the last 6 months
• 7 years of work history or 10 assignments documented
• Current vaccinations required: Hepatitis B and standard childhood vaccinations (proof of vaccination or positive titers required; declinations not accepted)
• Clean malpractice history (no settlements over $500K; no more than 3 total settlements)
• For telehealth roles: active licensure in state of residence and state where services are provided
