Primary Care Physician

PhysicianReports To: Medical DirectorDepartment: ProvidersSupervises: Physician Assistants, Nurse PractitionersLocation: This position is based in Fredonia NY but will also occasionally work in Westfield NYTravel Required: <10%Position Type: Full-time, ExemptThe Physician performs comprehensive physical assessments, provides services related to health maintenance and promotion, and establishes diagnosis for common short term and chronic stable health conditions. The Physician will perform other therapeutic or corrective measures as indicated and provide acute care when necessary, and within the scope of training. The Physician initiates appropriate action to facilitate the implementation of a therapeutic plan consistent with the continuing health care needs of the patient.Supervise the Physician Assistants and collaborate with Nurse Practitioners employed by the practice, as assignedParticipate in daily team huddlesAssess the health status of patients based on history and physical and a formulated diagnosis as a part of a team approach in caring for the individual patientProvide therapeutic intervention, prescribe drugs, devices, and immunizing agentsAssess patient systems, evaluate physical findings, formulate a diagnosis, and develop an appropriate management planOrder and review labs, diagnostic tests and provide appropriate follow-up of resultsMonitor and provide follow-up of clients with acute illness and chronic conditionsReferral of community resources that may be beneficial to patientsMake appropriate consultations and referralsUse Disease Management and Health Maintenance to coordinate care for individual patientsParticipate in clinical, educational, and administrative meetings and activitiesShare call schedule as directedRequired to maintain privileges at Westfield Memorial Hospital in WestfieldAny additional duties as assigned by managementWe are a caring and innovative organization.
