MDS Coordinator RN

A well-established skilled nursing facility (SNF) in Roselle Park, New Jersey is hiring a full-time MDS Coordinator (RN) . This is a dedicated MDS Coordinator role, not a floor position with MDS tacked on. The right MDS Coordinator here is a Registered Nurse (RN) who already knows the Minimum Data Set (MDS) process cold and wants a stable building where they can own it.

Prior nursing home experience is required. We are looking for an MDS Coordinator who has actually completed assessments in a skilled nursing / long-term care (LTC) setting and is comfortable with PDPM (Patient Driven Payment Model) reimbursement. What You'll Do

• Complete and submit accurate, timely MDS assessments under the Resident Assessment Instrument (RAI) process, including scheduling and tracking of all assessment types

• Coordinate with nursing, therapy, dietary, and social services to capture the full clinical picture for each resident

• Oversee care plan development and ensure documentation supports coding

• Monitor PDPM reimbursement accuracy and Quality Measures, and flag issues before they become problems

• Support survey readiness through clean, defensible MDS documentation

Qualifications

• Active Registered Nurse (RN) license in New Jersey

• Hands-on MDS Coordinator experience in a skilled nursing facility (SNF) / long-term care (LTC) setting is required
