Transportation Coordinator

Facility: Nassau Rehabilitation and Nursing Center Position: Transportation Coordinator Location: Hempstead, NY Schedule: Full Time | 8:00 AM – 4:00 PM Salary: $20.00 per hour Join Our Team! Nassau Rehabilitation and Nursing Center is seeking a reliable, organized, and detail-oriented Transportation Coordinator to coordinate non-emergency medical transportation for our residents. The ideal candidate will have excellent communication and customer service skills, strong attention to detail, and the ability to manage multiple transportation requests and schedule changes in a fast-paced nursing home environment. Key Responsibilities Receive, review, and coordinate non-emergency medical transportation requests for residents. Schedule transportation for medical appointments, including recurring and one-time trips. Verify trip details, including resident name, pickup and drop-off locations, appointment times, mobility needs, and required transportation level. Coordinate transportation for ambulatory, wheelchair, and other approved transportation needs. Communicate with transportation companies, insurance providers, medical offices, hospitals, clinics, dialysis centres, and other healthcare facilities. Monitor daily transportation schedules and follow up on pickups, drop-offs, delays, cancellations, no-shows, and schedule changes. Confirm transportation authorizations and required documentation when applicable. Enter and maintain accurate transportation information in applicable transportation management and facility systems. Assist with resolving transportation issues, including late vehicles, incorrect addresses, missed trips, and last-minute changes. Maintain accurate records of completed, canceled, and unsuccessful transportation trips. Communicate professionally with residents, families, drivers, healthcare providers, facility staff, and administration. Provide courteous and professional customer service while addressing transportation questions and concerns. Maintain resident confidentiality and handle protected health information in accordance with HIPAA and facility policies. Report complaints, incidents, accidents, safety concerns, or compliance issues to management. Perform additional transportation coordination and administrative duties as assigned. Qualifications & Requirements High school diploma or GED required. Nursing home, skilled nursing facility, healthcare, or medical office experience preferred. Previous experience coordinating transportation, scheduling appointments, dispatching, or working with medical transportation is a plus. Strong organizational and multitasking skills. Excellent telephone, communication, and customer service skills. Ability to communicate clearly and professionally with residents, drivers, healthcare facilities, insurance providers, and transportation companies. Basic computer proficiency, including email, data entry, Microsoft Office, and Outlook. Experience with SigmaCare or similar healthcare/facility software preferred. Ability to maintain accurate records and demonstrate strong attention to detail. Ability to work effectively under pressure and respond quickly to schedule changes and transportation emergencies. Ability to handle confidential resident and healthcare information appropriately. Professional, dependable, and team-oriented attitude.
