You’ll play a vital role in supporting patients through rehabilitation, chronic condition management, and senior care services. Key Responsibilities Provide skilled home health nursing care based on individualized care plans Support senior care and rehab patients in home and community settings Document patient visits, treatments, and progress in EMR systems Collaborate with physicians, RNs, families, and interdisciplinary teams Assist patients with chronic conditions, recovery, and dementia care Ensure compliance with California home health regulations Promote patient safety, independence, and overall well-being Qualifications Active California Licensed Vocational Nurse (LVN) license in good standing Graduate of an accredited vocational nursing program Minimum 1 year of nursing experience (home health preferred) Experience in senior care, rehab, or community care preferred Valid driver’s license, reliable transportation, and auto insurance Strong communication, organization, and documentation skills Experience with electronic medical records (EMR systems) What You’ll Gain
