Host Home Provider (HHP)Role summary: Ascendigo Autism Services is seeking compassionate individuals or families to serve as Host Home Providers for adults with autism and other intellectual/developmental disabilities.Host Home Providers open their homes to support an individual in a safe, stable, and inclusive living environment, while fostering independence, connection, and a meaningful life in the community.At Ascendigo, we believe in elevating the spectrum by empowering people, inspiring lives, and shattering expectations. As a Host Home Provider, you are not just providing a service, you are creating a home where someone is known, valued, and supported to live a full and meaningful life.Preference will be given to host homes located in Carbondale, Colorado; homes in surrounding Roaring Fork Valley communities may also be considered.Key Responsibilities:Daily Support & Skill BuildingProvide a welcoming, stable home environment with consistent structure and supportAssist with daily living activities (meals, routines, hygiene, household tasks) as neededFoster independence, confidence, and life skills developmentSupport behavioral, health, and wellness goals in alignment with the Individual Support PlanCommunity & ConnectionEncourage and facilitate community engagement, relationships, and meaningful activitiesSupport access to employment, recreation, and social opportunitiesProvide or coordinate transportation as neededCare Coordination & DocumentationPartner with the interdisciplinary team to implement person-centered supportsParticipate in ISP meetings and ongoing communicationMaintain timely and accurate documentation to support quality care and complianceUphold individual rights including privacy, dignity, choice, and autonomyFollow required incident reporting and communication protocolsHome Environment ExpectationsProvide a private bedroom and appropriate shared living spaceCreate an environment that promotes belonging, independence, and personal choiceMeet required safety and home readiness standards, including inspection and approvalPreference will be given to host homes located in Carbondale, Colorado; homes in surrounding Roaring Fork Valley communities may also be considered.Qualifications21 years of age or olderHigh school diploma or equivalentExperience with autism or developmental disabilities preferred (training provided)Valid driver's license, reliable transportation, and insuranceAbility to pass required background checks and screeningsStrong communication, organization, and relational skillsAlignment with Ascendigo's values:Heart – connection and encouragementMind – growth, learning, and inclusive thinkingVoice – honest, respectful communicationCompensation & StructureIndependent Contractor (1099)Monthly stipend based on level of care and individual needsAccess to training, tools, and ongoing supportOpportunity to build a meaningful, purpose-driven role from homeSupport from AscendigoComprehensive onboarding and trainingOngoing program oversight and partnership24/7 on-call supportAccess to clinical, behavioral, and program expertiseA mission-driven team committed to your successDESTINATION ASCENDIGO: Ascendigo's mission is to elevate the spectrum by empowering people, inspiring lives, and shattering expectations.
