Host Home Provider

SUMMARY: A Host Home Provider is an independent contractor who provides a safe, stable, and supportive home environment for an individual or small group of individuals with an intellectual and/or developmental disability. The Provider delivers person-centered supports that promote independence, community inclusion, and overall health and well-being in accordance with the individual's Person-Centered Support Plan (PCSP), applicable state regulations, and agency policies. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. - Provide a safe, clean, and supportive living environment within the host home - Deliver daily care and supervision tailored to the individual's needs, abilities, and preferences - Implement services and supports as outlined in the Person-Centered Support Plan (PCSP) - Assist with activities of daily living (ADLs), including but not limited to: - Personal hygiene and grooming - Meal preparation and nutrition - Medication reminders or administration (if authorized and trained, e.g., QMAP) - Promote community integration through social, recreational, and vocational activities - Support development of independent living skills and decision-making - Ensure the individual's rights are upheld in accordance with 42 C.F.R. § 441.301(c) and applicable Colorado regulations - Maintain a structured, respectful, and inclusive home environment Health, Safety, and Compliance - Ensure the home meets all applicable health, safety, and environmental standards - Follow all agency policies, including incident reporting requirements - Monitor and report any changes in the individual's physical, emotional, or behavioral condition - Respond appropriately to emergencies and follow established contingency plans - Maintain confidentiality of all Protected Health Information (PHI) Documentation and Reporting - Complete required documentation accurately and timely, including: - Daily service notes - Incident reports - Medication Administration Records (MARs), if applicable - Participate in care planning meetings and communicate regularly with the agency team - Maintain records in accordance with agency and regulatory requirements** ** ** Minimum Qualifications** - Must be at least 21 years of age - High school diploma or equivalent preferred - Experience working with individuals with intellectual/developmental disabilities preferred - Ability to pass required background checks, including: - Criminal history (CBI) - Adult Protective Services (CAPS), if applicable - Motor Vehicle Record (if transporting individuals) - Valid driver's license and proof of insurance (if applicable) - Current CPR/First Aid certification (or ability to obtain) - Completion of required agency training prior to providing services - Must provide liability insurance and have a business license Home Requirements - Provider must own or lease a residence suitable for providing care - Home must meet all licensing, fire safety, and environmental standards - Must have adequate space, furnishings, and accommodations for the individual - Willingness to allow home inspections and monitoring by the agency and regulatory entities Skills and Abilities - Strong interpersonal and communication skills - Ability to provide compassionate, patient-centered care - Ability to manage challenging behaviors in a calm and professional manner - Organizational skills and ability to complete documentation requirements - Commitment to maintaining professional boundaries while fostering a supportive home environment Work Schedule - This is a 24/7 residential support role with scheduled relief/respite as arranged - Flexibility is required to meet the needs of the individual Compensation - Compensation is based on the level of care and services au horized - Additional reimbursement may be available for approved expenses QUALIFICATION REQUIREMENTS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Must be able to pass a background check. ** ** STAFF SUPERVISORY RESPONSIBILITIES: May supervise one to two individuals. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities may include training new employees; planning and directing work; appraising performance; addressing complaints and resolving problems. EDUCATION and/or EXPERIENCE: High School diploma or general education degree (GED); equivalent combination of education and experience. ** ** LANGUAGE SKILLS: Ability to read, and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization. TECHNICAL SKILLS: Food preparation and other home management skills required. Computer skills to include the use of web based data entry systems for the purpose of program documentation to facilitate the agency's move toward a paperless environment. ** ** MATHEMATICAL SKILLS: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, decimals, and percentages. REASONING ABILITY: Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. ** ** CERTIFICATES, LICENSES, REGISTRATIONS: Must have valid Colorado driver's license and ability to meet insurance requirements. Must have ability to obtain Colorado Department of Institution Division for Developmental Disabilities Medication Administration Certification and First Aid/CPR Certification and other training as applicable. PHYSICAL DEMANDS: Daily duties may include but are not limited to: · Bathing, repositioning and changing of clients. · Transfer of clients from wheelchair to bed, from wheelchair to commode. · Assist clients while walking. This may include the use of gait belts. · Dealing with aggression (hitting, kicking, biting, etc.) · The use of crisis intervention techniques and stances. · Performing CPR/Heimlich if needed · Pushing/pulling wheelchairs · Lifting clients from weight range of 65 to 200+ lbs. ** ** Other frequent duties of this position include but are not limited to: Bending to complete cooking, mopping, vacuuming, cleaning bathtubs, toilets, ovens, etc. The employee is also regularly required to talk or hear and is frequently is required to stand; walk; sit; and use hands to finger, handle, or feel objects, tools, or controls. The employee is occasionally required to stand, walk, reach with hands and arms, climb or balance; and stoop, kneel, crouch or crawl; and taste or smell. WORK ENVIRONMENT: The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. The noise level in the work environment is usually moderate.
