POSITION OVERVIEW

The RN Case Manager is responsible for coordinating patient care across the continuum, ensuring safe and timely discharges, and collaborating with interdisciplinary teams including physicians, social services, and external providers.

ASSIGNMENT DETAILS

13-week contract Day shift | 5x8s (08:00–16:30) Weekend rotation required Patient ratio: approximately 1:20 Acute care hospital setting Epic EMR No floating required

COMPENSATION

Gross: $2,752 per week W2: $24.00 per hour Tax-Free Stipend: $1,792 per week

CORE RESPONSIBILITIES

Coordinate inpatient care and discharge planning Perform utilization review using InterQual criteria Collaborate with interdisciplinary teams and external providers Arrange DME, SNF placement, and transportation services Ensure compliance with payer and regulatory guidelines Document accurately in Epic EMR

REQUIREMENTS

Minimum 2 years acute care Hospital Case Manager experience (HARD STOP) Experience with Care Coordination and Discharge Planning InterQual criteria experience Epic EMR experience Travel experience Active California RN license Ability to coordinate DME, SNF placement, and transportation
