The Nurse Care Coordinator shall provide regional primary care consultation, psychiatric care consultation; work with the Behavioral Health Home Organization (BHHO) & Certified Community Behavioral Health Clinic (CCBHC) team, Child Assertive Community Treatment Team, Private Non-Medical Institute, and other members to facilitate care across the healthcare continuum.

Rate: $33 - $36 depending on experience

ESSENTIAL FUNCTIONS:

• Provide educationand support around medications to the Interdisciplinary Team (IDT),members, and families.

• Provide medicaleducation about chronic disease management to IDT members, families andgroups.

• Liaison to HealthHome, ACT Teams, CCBHC Practices/Primary Care Physicians.

• Collaborate withthe IDT on member plan of care.

• Review HealthQuestionnaire and provide recommendations to IDT and/or members andfamilies for medically compromised participants.

• Assist, and/ormake recommendations regarding skill development in activities of dailyliving, including hygiene, nutrition, exercise, general healthmaintenance/improvement.

• Supportimplementation of grant funded projects such as In-Shape, CTI, and otherhealth and wellness pay for performance initiatives in geographical areawhere program is offered.

• SupportBehavioral Health Home team, CCBHC and Child ACT in achievingorganizational and state required outcomes, including but not limited toA1C testing, tobacco screening and annual health screenings.

• Be proficient inEHR, HIN, and VMS systems in order to extract clinical data and alerts inorder to support level of care.

• Consult with IDTand members as indicated regarding discharge planning recommendations andfollow up.

• Monitor emergencydepartment usage and coordinate with IDT and other providers to promoteleast restrictive level of care.

• Medicationreconciliation through EHR from external providers (i.e. PCP, Specialists,etc.) when appropriate.

• Providemedication and medical education to Interdisciplinary Team (IDT), members,and families.

• Complete pharmacyenrollments, medical equipment orders, and other supports/services tosupport recovery goals.

• Provide CRMATraining and Supervision as needed.

• Schedules will varyaccording to program needs (as applicable): On-call responsibilities asassigned

EDUCATION:

• Graduate from an accredited school of nursing.

CERTIFICATES, LICENSES, REGISTRATIONS:

• CurrentRN license in the State of Maine

• Maintaina valid state driver's license.

EXPERIENCE:

• Previousexperience working with adults with serious and persistent mental illnesspreferred.
