The Care Coordinator - Behavioral Health role provides direct outreach and linkages to community resources to consumers/clients in assigned programs. Guides clients, consumers and their families through the care continuum establishing contact upon referral and continuing contact throughout care and to self-management and/or active treatment.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

• Education: Bachelors Degree in Social Work or related field preferred.
• License/Certifications: N/A
• Experience: Two years of experience working with behavioral health consumers and families preferred. Experience in a team based setting and collaborating with multiple providers preferred.
• Additional Skills/Requirements Required: N/A
• Additional Skills/Requirements Preferred: Knowledge about community resources.

The base range is an estimate and does not reflect the full value of our total compensation package.

The pay range for this position is $52,499.20 to $71,052.80 per year.

Compensation for part-time positions is determined on a prorated basis consistent with the scheduled hours and applicable full-time equivalent (FTE).
