POSITION SUMMARY Case Manager - Typically placed on Med-Surg or Tele floors following Internal Medicine and Family Medicine. RESPONSIBILITIES Knowledge of discharge planning and making referrals. Typically work on a Medical-Surgical (MS) or Tele unit and split the floor with a Social Worker. function independently on the unit and collaborate with an interdisciplinary team and patients for discharge planning. Round on the unit with physicians and the team. Maintain a positive attitude, think critically, and have strong time management skills. Utilization Review (UR) is a separate department responsible for insurance approvals and arranging transportation. MINIMUM QUALIFICATIONS 1 year of minimum experience. BLS Certification. PRI (Patient Review Instrument) Certification. SCREEN Training Certification. ADDITIONAL CERTIFICATIONS PRI & SCREEN certifications can be taken online through the School of Albany; typically take a couple of hours for each certification. Cost is approximately $300 per certification. Reference: LENGTH OF ASSIGNMENT Length of assignment not specified. SHIFT / HOURS PER WEEK 37.5 hours per week. SYSTEMS Allscripts #J-18808-Ljbffr RN.com
